Recent research suggests associations between poor oral health status and pulmonary diseases such as pneumonia. This presentation will review the background information that suggests that oral bacteria influence the course of lung infection, and that improved oral hygiene may help reduce the risk for pneumonia in high-risk populations. Results will be presented from a recently completed pilot randomized, placebo-controlled trial that evaluated the frequency of use of oral topical chlorhexidine gluconate (CHX) to prevent or reduce oropharyngeal colonization by respiratory pathogens in mechanically ventilated-intensive care unit (MV-ICU) patients. Periodontal Disease and Atherosclerotic Vascular Disease

The association between periodontal disease and cardiovascular and cerebrovascular diseases has been demonstrated by many studies. These studies include about 20 cross-sectional studies, and 23 cohort or longitudinal studies. With few exceptions, they show an association of periodontal disease with atherosclerotic vascular disease (ASVD) with odds ratios of between 1.2 to 6.5, and hazard ratios of similar magnitudes. It is important to note that most of these studies statistically adjust for possible confounding, some of which may increase the risk for both diseases.

Saliva is critical for maintenance of good oral health. Key actions of saliva include: buffering to prevent an acidic oral environment which predisposes to dental demineralization and supply of calcium and phosphate, key substrates for dental remineralization, and protein secretion that enhances the local defenses against infection. Dry mouth can develop due to salivary gland hypofunction or due to pathological dysfunction of the glands. In the general population there are about 10% of individuals with dry mouth. In individuals over 65 years of age, about 20-25% complain of dry mouth. In nursing homes up to 50% of individuals can have dry mouth.
