Dental history
- History of lost tooth due to trauma (tooth 11 lost at 10 years old)
- History of fear of dental treatment
- Oral hygiene:
- Brushes 2x/day with a manual toothbrush for about a minute using fluoride toothpaste, using sweeping motion side by side
- Flosses 1x/day at night using string dental floss, but does not clean under the bridge because patient states it is too difficult
- Notices bleeding of the gums almost every time when brushing
- Does not use any mouth rinse or high-fluoride toothpaste
- The patient is not happy with his smile and the appearance of his teeth
- History of multiple cavities, fillings, root canals, crowns, bridges
- Does not wear his existing lower partial removable denture due to discomfort
- No sensitivity to cold, hot, sweets, or pressure
- No oral habits including clenching, bruxing, grinding
Cultural and socioeconomic factors
- During his childhood, grew up very poor and stated that he did not get enough nutrients including calcium and milk

“Now I have all the money but it’s too late to gain my oral health back because I have already lost some teeth”

- There was a lack of awareness of preventative measures that can be implemented to stop caries progression and stabilize the periodontal disease
- Patient usually does not seek dental treatment first until a student contacts him to book an appointment for an examination or cleaning, even though he has some discomfort and pain in his teeth
- The patient is usually told what treatment needs to be done and believe those would solve all the problems, however, long-term maintainability goals were not addressed and achieved

Due to the patient’s hay fever and chronic coughing, he sometimes does not feel comfortable coming to the dental office because others may see him as sick and spreading a cold, therefore he mostly avoids visiting during allergy season
- The patient also wears dental gloves throughout the whole appointments

When patient was asked what his long-term goal was, patient stated “to stay healthy and prevent early death”
- The patient stays active and exercises regularly to stay fit, however, there is a lack of dietary knowledge and education that may negatively impact his physical and oral health
- The patient usually eats out or buys take-outs and whenever he is watching TV he eats a lot of junk food that is convenient and drinks a lot of juice
- Also, when the patient needs to take medication at night due to eczema, he eats some food but does not always brush after because he “gets lazy”

- Showed how to use each floss with patient holding the mirror, advised patient to try all of them and see which one is more comfortable and convenient

Instructions on how to clean the denture:
- Remove denture from the mouth and rinse with warm water
- With a denture brush, gently brush your denture using a non-abrasive denture cleaner to remove plaque and debris then rinse again with warm water
- Soak the denture in denture-cleaning solution overnight and do not let the denture dry out which may cause it to become more brittle
- Do not wear the denture when sleeping
Patient outcomes

Beginning from a thorough comprehensive examination to developing a personalized treatment plan that not only addresses his current dental needs but also setting SMART goals for the long term management of his oral health was an imperative journey for myself and the patient
- Although patient has been a patient for a long time at UBC, his long history of caries and frequent restorative procedures was an indication of inadequate caries management including oral hygiene practices, preventative treatment and low sugar diet

The first treatment that was offered was making of his flipper denture as he was psychologically distressed without his front tooth which negatively affected his quality of life
- During these appointments, I was able to build rapport with the patient and get to know him personally which aided in creating a personalized treatment plan
- It was important to actively listen to why it has been difficult to keep up with good oral hygiene and come up with solutions that work for him in the long term
- Every risk factor for caries including poor oral hygiene, infrequent cleaning by dental professionals and high sugar consumption was discussed with the patient

Once the patient was satisfied with the flipper denture and denture hygiene instructions were given, I visually demonstrated proper brushing techniques with dental care model and large toothbrush used for educational purposes
- Disclosing solution was used to show the patient the area of high plaque and calculus accumulation and the patient was surprised because even though he brushed before coming to the appointment, there were a lot of “pink spots”
- Visual aids were much more effective in educating the patient about oral hygiene instructions and effectiveness of his brushing method and patient said these would help him remember what was emphasized

During scaling and root planing, there was a lot of bleeding as the gums were inflamed and heavy subgingival calculus was difficult to remove
- Few areas with deep pockets and subgingival calculus had to be anesthetized for patient comfort and through debridement
- Took two appointments for cleaning and in every appointment oral hygiene instructions were re-emphasized

Overall, patient was highly satisfied with the educational information that was presented to him and became more motivated to change his habits to build oral health
- During the re-evaluation stage 6 weeks after the completion of debridement, patient had decreased probing depth, BOP, and increased oral hygiene effectiveness
- The patient stated that he has been using Proxabrush to clean underneath the bridge because it is the quickest and easiest
- He has also been drinking more water and chewing xylitol gums once in a while and feels his mouth is less dry than before
- Once phase I is completed, need to make sure that his periodontal state is stabilized before moving on to phase 2, which the patient understood
- Advised the patient that he needs to be more active in visiting dental offices and calling the office when he is due for a cleaning or when he has dental pain rather than waiting

The most valuable lessons I have gained through this patient interaction:
- Involving patients in the decision making process allows them to participate actively in managing their oral health while improving their compliance and chance of successful treatment outcomes such as giving the patient a few options of methods of flossing under the bridge and allowing the patient to determine what is most beneficial and efficient
- The importance of preventative education about oral diseases and considering patient’s personal socioeconomic and cultural factors that can create barriers to maintaining oral health
- Building achievable goals with close up follow-up and monitoring of periodontal status and oral hygiene effectiveness with implementing different strategies
